Required Documents
The following documents must be submitted in full for the enrollment process:

• Student Information and Enrollment Form (to be completed by the institution)

• Kindergarten Enrollment Agreement (to be signed mutually at the institution)

• Copy of identity card

• Copy of vaccination card

• Medical report from a pediatrician stating that the child has no health condition preventing kindergarten attendance (can be obtained from public or private institutions)

• 12 passport-sized photos of the student

• 3 A5-sized passport-type photos

• A5-sized family photo with family members

• A5-sized separate photos with mother-child and father-child

• A5-sized baby photo

• Photos of persons authorized to pick up the child from school other than parents or legal guardians

Required Items
To meet the children’s daily needs, please provide the following items labeled with the child’s name:

• Pajamas and bedding set (not required for 6-year-old group students)

• Quilt and pillow (with mattress protector)

• Indoor shoes (without laces) or slippers

• Comb or brush (in a box, labeled)

• Wet wipes (labeled)

• Undershirt, socks, underwear, vest, and one spare outfit (top and bottom)

• Small backpack (for spare clothes, also used during field trips)

• Fever reducer or special medication for emergencies (with explanation label)

• Toothbrush with cap (for ages 4+, toothbrushes are recorded and renewed every 3 months)

• Herbal, child-safe toothpaste (toothpaste should not be sent for children under 4)

• A box of various hair accessories (for girls)

Important Note
A short note about your child’s sleeping habits, eating routines, special conditions, hobbies, or phobias must be prepared and submitted at enrollment.
All items brought to kindergarten should be delivered in a labeled bag.
